구강편평상피세포암 증례들의 예후예측에 있어서 Treatment-dependent Prognosis Index (TPI)의 적용

Abstract
결과
서울대학병원 구강악안면외과의 편평상피세포암으로 진단받은 179명의 증례들을 대상으로
하여 D SAK의 Treatment-dependent prognosis index (TPI)를 기준으로 임상적 요소에 따
라 분류하고 치료군과 치료효과에 따른 예후의 지수(index)를 구하여 생존곡선을 산출하고
실제생존곡선과 비교하여 다음과 같은 결과를 얻었다.
1. 임상적 요소의 조합에 따른 48가지의 조합중에서 17종류 조합의 증례들이 해당되었고
E38증례가 가장 많았으며 E44, E37, E14 E39의 순이었다. 결론적으로 D SAK연구와 본 연
국의 결과를 비교해보면 D SAK연구에서는 E14, E2등의 초기병기의 환자들이 많고 근원적
(radical)한 치료후 완전한 병소의 제거가 이루어진 증례가 많은 반면 본 연구의 결과에서는
E38, E44등 보다진행된 병기의 환자들이 많아 근본적인 치료도 시행되지 못하고 치료후 완
전한 병소의 제거가 이루어지지 못한 증례가 많았다.
2. 192가지의 가능한 예후중에서 본 연구에서는 38종류 예후의 index가 해당되었으며, 그
중에서는 E38-B-ED의 증례가 가장 많았고, 그 다음은 E38-A-NED, El4-A-NED,
E44-B-ED, E37-A-ED의 순이었다.
3. 본 연구의 생존곡선 비교는 군의 크기가 비교할만큼 크지못해 유의성의 유무에 관해서
는 결론을 내리지 못하였으나 E38-B-ED의(Fig. 1) 증례에서는 그 형태가 실제생존곡선과
TPI로 구한 생존곡선이 비슷한 양상을 보였다.
4. D SAK의 TPI의 국내환자들에 대한 적용가능성유무는 모든 그룹별로 환자의 수가 부
족하여 생존곡선에 대한 유의성 판정이 불가능하여 국내 구강암환자들의 경우 D SAK연구
의 TPI에 의존한 정확한 예후 판정은 불가능하나 각 증례그룹별로 곡선의 형태가 매우 유
사한 형태를 보이고 있어 간접적인 예후 추정에 유용할 것으로 사료되며 향후 보다 대단위
의 임상적역학조사연구가 진행되어진다면 보다 정확하고 유효한 자료가 될 수 있으리라 추
정된다.

Applied TNM-system for determination and prognostic classification of the cancer
patients of Oral and Maxillofacial region does, however, only partly justice, if at all, to
the nature of the tumor disease. And the TNM-system implies among other defects a
crude simplification of the complex pattern of the tumor disease. But in clinical practice,
the prognosis must be determined on the bases of a specific combination of clinical
factors under consideration of various therapeutical conditions.
For more reliable and objective prognosis determination for the cancer patients of Oral
and Maxillofacial region, 'Retrospective D SAK(German-Austrian-Swiss Association for
Head and Neck Tumors) study' introduced Treatment-dependent Prognosis Index TPI in
1982. Treatment-dependent Prognosis Index TPI gives the clinician and opportunity to
determine the prognosis before starting therapy at the time of the first admission of the
patient, during the therapy and immediately after complete of the therapy.
The fundamental purpose of the TPI can be summarized as follows. 1) In
clinical-therapeutical cancer research the TPI provides a sound basis for the planning
and verification of therapy studies. 2) In clinical practice the TPI allows a reliable and
objective determination of the prognosis for the individual patient and thus provides the
clinician valuable assistance in planning the treatment.
The authors estimated the survival curves of the 179 cases of squamous cell
carcinoma from 1985.1.1 to 1992.8.31 in the Department of Oral and Maxillofacial
Surgery, Seoul National University Hospital according to Treatment-dependent Prognosis
Index TPI of Retrospective D SAK study and compared with the survival curves
observed in long term study on prognosis of patients.
